Applying for a Dependent's ID

The Office of the Adjutant General (OTAG), AFP implements the AFP ID System.  Requirements for securing a Dependent's ID are enumerated below, together with the links for downloading the application forms.

Requirements for Issuance of AFP ID for Active Military Personnel's Dependent
  1. Application form* duly accomplished properly endorsed by the Admin Officer of military personnel.
  2. For Spouse - authenticated copy of Certificate of Non-Marriage issued by NSO
    For Children - authenticated copy of Birth Certificate issued by NSO
    For Parents - authenticated copy of Marriage Contract of parents and Birth Certificate of military personnel (for married military personnel, age of parents should be 60 yeras old and above to be qualified).
  3. ORDERS: CAD/ETAD, Enlistment/Re-enlistment, Promotion, Assignment, Change of Branch of Service, Change of Marital Status, Amendment Orders, whichever is applicable.
  4. Surrender old AFP ID. If lost, attach Affidavit of Loss and Police Blotter.
  5. Present any other valid ID.

              Note:   ID picture to be attached in the application form must be most recent 2x2 colored picture with white background in formal/semi-formal attire.

Requirements for Issuance of AFP ID for Reservist's Direct Dependent
  1. Application form* duly accomplished properly endorsed by the Admin Officer of Reservist.
  2. For Spouse - authenticated copy of Certificate of Non-Marriage issued by NSO
    For Children - authenticated copy of Birth Certificate issued by NSO
    For Parents - authenticated copy of Marriage Contract of parents and Birth Certificate of Reservist (for married military personnel, age of parents should be 60 yeras old and above to be qualified).
  3. ORDERS: Appointment, Promotion, Unit Assignment, Amendment, whichever is applicable.
  4. Surrender old AFP ID. If lost, attach Affidavit of Loss and Police Blotter.
  5. Present any other valid ID.

           Note:   ID picture to be attached in the application form must be most recent 2x2 colored picture with white background in formal/semi-formal attire.
